I am pretty sure they are removing a titanium rod from the tibia of a person who had had a very serious lower leg break.

I second #7. Yes, the tool is made for that. You can see the strike plate on the end of the removal device as well as the pull handle at top.

I'm a surgeon.
This is a standard procedure. What you see on the video is a follow up to stitching.
Device inside patients body is called tightener and one held by the doctor is called "the hammer". What you see happening over there is a standard procedure of tightening stiches freshly applied to a damaged skin tissue.
Subject on the video suffered a cut to his skin caused by improper use of washing mashine. This resulted in an operation of putting TWO stiches in the knee area of his leg.
