The large white and black quarry throughout the video is Rock of Ages Quarry in Barre, Vermont.
The old steel bridge at the beginning is the bridge at Livermore Falls in Campton, NH. A friend in high school died falling off that bridge. That bridge is no joke. There is nothing but steel beams to walk on. Very dangerous. At :49 you can see the beach on the river that was a great swimming spot in my childhood.

#3 I haven't been there in a long time, but last I knew it wasn't a working quarry and you weren't allowed access because with all the water and limited ways of getting out it was too much of a liability.
