Yes it is real, the "burning" in your mouth from different peppers is poison in fact the poison attacking. There you see the effect of a to strong poison..

throughout the whole thing it seemed to be real but what is up with that last image?! maybe he was scared to leave the house, but i'd be petrified to stay. if i had tasted it i wouldn't want to be alone! and what the hell was he thinking having only milk as precaution?!

The end is not real for sure.... hot peppers can cause skin damage, but he'd have got to rub the pepper all over his face... I worked in a kitchen before where a cook seeded Jalapeno peppers, which are not even close to these ones, after 5lb of seeded jalapeno, he went to the bathroom without washing his hand touching his wii... well guess what... he had a hot sack... no scab or anything gross, but he said that it hurt quite a bit!!!

#14 The reaction at the end is real. http://en.wikipedia.org/wiki/Urticaria . The reaction spreads all over the body regardless of where the allergen is introduced. I've had this happen to me although with a different cause. Lucky his throat didn't swell and cut off his breathing.

#16 I think the fact that he didn't get a swollen throat is because he only bit the first end of the pepper which isn't as bad.. Because as you will know, the hottest part of any peppers are the seeds inside it.
Still you could still go further with this madness and just try pure capsaicin (the chemical in peppers, which has the burning effect). For comparison, pure capsaicin rates at 15,000,000–16,000,000 Scoville units.

#16 It doesn't say anything about being caused by pepper. And besides that there are plenty of other videos from people who eat it too and don't get such a reaction. I'd say it's fake. But I'm also a little bit impressed by that guy, because I can't imagine how hot that must be .

his face around 2:16 seemes abit fake too? iv seen people eat hot peppers (not these tho) and not had a reaction like that. or is that purely because of the 1mil count?

If he faked the outcome a.k.a. skin burn, then he faked the pepper too. It's fairly easy, if you've got imagination.

Doesn't anyone find it interesting that the pepper in the picture shown looks nothing like the pepper he is holding (then eats)??
The pepper in the picture is lumpy, deeper red and short. The one he eats is slender, dark orange in color, and smooth looking. Hmmm.
I mean, that pepper he ate was probably super hot too, but something is a little strange here.

Secondly, as a fan of hot peppers, I'm surprised he was able to talk as clearly. I've had some reasonably hot ones (usually by mistake, and nothing as severe as he's talking about) and I couldn't speak intelligible English if my life depended on it. There's usually a lot of "Oh, s**t, ow, ow ow, f**k, ah! ah! that's hhhhhhhhhhooooottttttttt!!!!!!" etc, etc instead of reminiscing about the birth of your first child.

#29 - That makes a lot of sense. I've eaten much milder things and it's disrupted my voice.
I'm 100% convinced that this was meant to be a parody. The images at the end, where he's all burned up, is certainly not hives.
Mainly because if you compare his facial hair from before, it's a light brown. In the final images it's practically black, like it's been burnt to a crisp.

#14 been there done that, no idea what peppers those were, was just helping out with the Thai cooking, cutted seeded peppers, and yes it hurts... it's durable but you'll feel it for over an hour! (and I DID WASH MY HNDS WITH SOAP AFTER I CUTTED THEM, so if it hurts your sack, don't even try to touch your eyes!)
