2 faults. Load was not secured and safety distance too short. Chain of just two errors contributed to this accident. If you take out any of the faults made, no accident. So when you are driving and neglect road rules, you should always consider others also.

I agree with #6, the towing car needs to be in the right lane...poor motorcycle driver..:(
