#1 Some sort of string that he holds in place with his mouth, you can see his head doing movement that fits with the "flying".

#3 if he holds the string in his mouth, how can he make the card go all the way around him without entangling it around his face?
I think the string being taped to the ceiling is a better explanation.

#5 He can loop it around his neck once, then do it the other way next time, making it seem like he loops several times, but never more than one revolution. The reason i believe its this way is because we had toys that were similar when i was little, where you had a thin line and a lil furry thing on the end, and it was pretty common to use your mouth to handle it, making it seem like it crawled around.

I don't think there was any string involved in this trick. There were many techniques involved including obviously great spinning and throwing, but the biggest part of this was using magnets. If a quality playing card is held between two attracting magnets for several minutes, it will become magnetized, if then a magnet is attached/glued to the palm of the hand or hands the card will behave very interesting when it approaches the hand magnet allowing hovering and slow motion effects. The other effects were largely down to many many many hours of practice.
