Why didn't someone just cut the sound off whilst they sorted their 'technical issues'.

I can totally understand why he was so angry :P that would have pissed me off too. And yea she was talking as if nothing ever happened so she's probably used to it and thus I agree with #3

My wife worked for a news studio once. Apparently when something like dead air happens everyone goes into a huge panic. Every second has been measured to cost huge amounts of money.

It was fake, you can tell that someone recorded a voice over the original. The sound was coming from two different places, you can hear it.

could be fake, but maybe the recording was separate and someone who worked there made a version with the two combined. if it is real the guy was probably yelling because of the person responsible for the audio glitch

This is called the Director's Track, or Producer's Track. It's only audible to those in the studio and the surrounding technical areas, and only over headsets. It doesn't go out over the broadcast at all.
