As for the chubbiness, I guess he could use a bit of control but he's at a growing age, so it shouldn't be too hard.

After it's been established that the kid has an illness I cannot fathom what moves people to still comment on his appearance.
Furthermore the Internet is two-way traffic so it might be a good idea to breathe in and out two times and think what you might feel like when someone wrote about you a similar remark that you have in mind.
